Suwannee Medical Personnel Provides Exceptional Quality Care Through Multidisciplinary Patient Centered Programs and Transitional Care Planning
Transitional Care Management includes the complete consultation and communication of our In-Home Care Team to assess and provide continual care of the patient’s changing medical needs upon discharge from a hospital or rehabilitation facility.
Health, Balance, and Wellness Programs include Physical Therapy and Occupational Therapy to improve strength and endurance, gait and balance instruction while promoting independence and a sense of well being through exercise, nutritional guidance and lifestyle changes.
Complete Care Coordination of Cardiovascular Health, Diabetes, Wound Care, Medication Management, Pain Management, Anti-Coagulation Therapy and Nutritional Education.
